Download the Affidavit or Declaration Form

Here are the Form Affidavit and Form Declarations that need to be submitted to John Tiedt for inclusion in the Kimkins.com lawsuit.

If you are NOT a California resident, download the Form Affidavit, fill in your name, payment info, etc. and take the form to be notarized at your local bank, etc. prior to mailing.

If you are a California resident, download the Form Declaration, fill in your name, payment info, etc. and mail.

P.S.  Heidi (as Noelle) is in the midst of issuing refunds to those who want them.  Please send the money back through paypal or reject the refund and participate in the lawsuit!
